Please complete the Coaching Agreement, Personal Health Plan, and Coaching Session Prep Form before your first session. Before each future session please complete the Coaching Session Prep Form. These forms help streamline our work together and keep us focused on your health goals.
1. Coaching Agreement
The Coaching Agreement lays the groundwork for our partnership. It defines the roles, expectations, and boundaries of our coaching relationship, ensuring clarity and alignment. By signing this agreement, you commit to the process of health coaching, and I, as your coach, commit to providing guidance, support, and accountability as we work together to achieve your health and wellness goals.

2. Personal Health Plan
The Personal Health Plan is your roadmap to whole-person health and well-being. This form guides you through a self-assessment of your current health status and invites you to define clear goals across key areas such as nutrition, fitness, sleep, and mindfulness. Through this reflection, you’ll identify specific actions to help you move toward your vision of optimal health. This plan is tailored to your unique journey and is regularly reviewed to keep you on track toward your long-term wellness objectives.
3. Coaching Session Prep Form
This form is a critical tool for both new clients and ongoing sessions. For new clients, it helps establish a clear starting point by encouraging reflection on your current health status and goals. For ongoing sessions, it serves as a checkpoint to monitor your progress, successes, and any challenges since our last meeting. By completing this form prior to each session, we ensure that our time together is focused, productive, and aligned with your evolving health and wellness objectives.
